For more than half a century, the public health crisis impacting people with SMI has often been ignored, and until recently, rarely addressed. Compared to people without mental disorders, people with SMI die at a younger age. Efforts to improve the health of people with SMI need to move beyond focusing on individual behaviors and personal agency and carefully consider and examine the social conditions that impact the health and healthcare of this population. We need to raise our analytical and intervention gaze to consider that health behaviors do not happen in a vacuum, they are greatly influenced by the structural forces and social conditions that shape people's live circumstances and choices. The information from the case studies in this text come from different studies over 10 years and are a composite of different people. To protect their identity and confidentiality, fictitious names are used. Details such as place of birth, geographical location, age, gender among others are also fictitious, to ensure anonymity--
